Attention to all Lower Sioux Community Members: The 2016 health insurance cards will be available for pick up during the Clinic Open House for Community members on Thursday, December 10th from 4:30 p.m.- 7:30 p.m. at the Lower Sioux Health Care Center.
- Each community member is required to sign for their insurance cards due to change of insurance companies from Blue Cross Blue Shield to CCS tpa. Due to the insurance change, no insurance cards will be mailed out.
- If you are unable to attend the open house, insurance cards may be picked up in the Health Department offices. Please see Lower Sioux Benefits coordinator, Chris Helget with any questions.
