Adult Services
Health Care Assistants
Health Care assistance is available to Elder’s living alone who have no one to assist them in their home and have difficulty performing daily tasks. Services provided may include visiting, light housekeeping, such as sweeping, mopping, light dusting or vacuuming.
Home Health Visits
The community health nurse, in collaboration with CWOO, will do home visiting. Home health visits inclusive of blood pressure, vital signs, health promotion, health education, medication management, clarification of hospital discharge instructions. Skilled Nursing services should be set up through a home care agency. The home care agency may contact Lower Sioux Community Health if additional support services are needed.
Medication Management & Set-up
Medication Management is available to Community Members who take multiple medications. The Community Health Nurse will help you categorize your medications; clarify dosages and times the medications are best taken. The Community Health Nurse can set your medications up for you as well.

Lactation Services
The Lower Sioux Community Health Nurse is a Certified Lactation Counselor and may be contacted at 507-697-8940 for support on your breastfeeding journey.
Health Education
Family Spirit
The Family Spirit Program is currently the largest, most rigorous, and only evidence-based home-visiting program designed specifically for Native American families. Over 1,000 Native families have received Family Spirit services since its inception. The program is designed to be delivered by Native American paraprofessionals as a core strategy to support young, Native parents from pregnancy to 3 years post-partum. It is an honor and privilege to provide this comprehensive program to interested families.
Program Objectives
- Increase parenting knowledge and skills
- Address maternal psychosocial risks that could interfere with positive child rearing
- Promote optimal physical, cognitive, social/emotional development for children ages 0-3
- Prepare children for early school success
- Ensure children get recommended well-child visits and health care
- Link families to community services that address specific needs
- Promote parents' and children's life skills and behavioral outcomes across the lifespan
How often do we meet?
The Community Health Nurse will do home visits with you weekly, monthly, or whatever your schedule allows. You can follow the specially designed curriculum of 63 lessons or choose just the lessons you want to learn about. You will be offered visits from early pregnancy until your child's third birthday.

Eligibility Requirements
- Lower Sioux Community Member or spouse/girlfriend of LSIC member.
- Any woman who is pregnant and/or has a child younger than 3 years old can participate. It is especially useful for young mothers under the age of 22 or at-risk mothers

Public Health Emergency Preparedness
The program is responsible for providing emergency preparedness training and also to increase the capabilities of the LSIC to respond to public health threats and emergencies, such as natural disasters, severe weather, bioterrorism, disease outbreaks, mass casualties, and chemical or radiation emergencies by assisting in the development of plans and policies.
Program Provides:
- Education and training on family emergency preparedness
- Risk communications and health information dissemination, monitors information to assure the most current and accurate information is available.
- Monitor state health alerts
- Coordinating LSIC preparedness and response to public health emergencies
- Participates in the development, review, and management of policies for preparedness to mitigate possible events that could jeopardize public safety, health, and the environment.
